True vertigo — the sensation that the world is spinning — almost always points to an inner-ear or central vestibular cause.
Common culprits include Benign Paroxysmal Positional Vertigo (BPPV), vestibular neuritis, Ménière's disease and vestibular migraine. A careful history and bedside tests usually identify the cause within a single consultation.
Many conditions respond beautifully to in-clinic manoeuvres such as Epley's repositioning, vestibular rehabilitation exercises and lifestyle modification. Medication is used judiciously — never as a long-term substitute for definitive treatment.
